Overview

Anti-aging peptides target the cellular mechanisms of aging: telomere shortening, mitochondrial dysfunction, accumulation of senescent cells, and declining NAD+ levels. This category represents some of the most exciting — and most serious — research in longevity medicine. Compounds like Epitalon, NAD+, MOTS-c, SS-31, and FOXO4-DRI work through distinct pathways that collectively address the hallmarks of aging at the cellular level.

Epitalon

Epithalon, Epithalamin tetrapeptide

Mechanism

Tetrapeptide that activates telomerase, the enzyme that maintains telomere length. Also stimulates melatonin production by the pineal gland and acts as an antioxidant. One of the few compounds with documented human telomere elongation data.

Benefits

  • Telomere elongation
  • Melatonin regulation and improved sleep
  • Antioxidant effects
  • May extend cellular lifespan
  • Neuroprotective

Dosing

500–1,000mcg per day × 10–20 days. Run 2–4 cycles per year.

Reconstitution

10mg + 3mL BAC = 3,333mcg/mL.

Cycle Length

10–20 day cycles, 2–4 times per year. Off periods allow telomerase activity to normalize.

Timing

Morning or before bed. Can combine with DSIP for sleep cycle.

Stacks Well With

MOTS-c, SS-31, NAD+, DSIP (for sleep)

Cautions

Well-tolerated in research. No significant adverse effects reported. Long-term effects unknown.

NAD+

Nicotinamide Adenine Dinucleotide

Mechanism

Essential coenzyme involved in hundreds of metabolic processes. Declines significantly with age. Required for DNA repair, mitochondrial function, sirtuin activation, and cellular energy production.

Benefits

  • Immediate energy boost
  • DNA repair support
  • Sirtuin activation (longevity pathways)
  • Mitochondrial function
  • Cognitive clarity
  • Reduced neuroinflammation

Dosing

25–100mg injectable. Start low (25mg) and assess tolerance before increasing.

Reconstitution

100mg + 2mL BAC = 50mg/mL. 500mg + 3mL BAC = 167mg/mL. 1000mg + 3mL BAC = 333mg/mL.

Cycle Length

Can be used 2–5x weekly. Some researchers do loading protocols (daily × 5 days then 2x/week).

Timing

CRITICAL: Inject slowly — minimum 3–5 minutes per mL to prevent chest tightness, anxiety, and flushing. IM or slow SubQ.

Stacks Well With

MOTS-c, SS-31, Epitalon (complete mitochondrial/longevity stack)

Cautions

Must inject slowly — rapid injection causes significant discomfort (chest pressure, flushing, anxiety). These effects are dose-dependent and resolve quickly but are distressing.

MOTS-c

Mitochondrial Open Reading Frame of the 12S rRNA-c

Mechanism

Peptide encoded in mitochondrial DNA. Acts as a mitochondrial hormone, improving insulin sensitivity, activating AMPK, and mimicking some effects of exercise. Improves metabolic flexibility.

Benefits

  • Exercise-mimicking metabolic effects
  • Improved insulin sensitivity
  • Weight management support
  • Mitochondrial health
  • Reduced age-related decline

Dosing

5mg, 2–3x per week. SubQ or IM.

Reconstitution

10mg + 1mL BAC = 10,000mcg/mL. 30mg + 3mL BAC = 10,000mcg/mL.

Cycle Length

Can be used long-term. Common: 8–12 weeks on, 4 off.

Timing

Pre-workout timing may enhance metabolic effects.

Stacks Well With

SS-31 (mitochondrial stack), NAD+, Epitalon

Cautions

Well-tolerated. No significant adverse effects in research.

SS-31

Elamipretide, Szeto-Schiller peptide 31

Mechanism

Targets the inner mitochondrial membrane, reducing oxidative stress and improving mitochondrial morphology. Protects mitochondria from dysfunction caused by aging and disease.

Benefits

  • Reduced mitochondrial oxidative stress
  • Improved energy production
  • Heart and kidney protection
  • Slows age-related mitochondrial decline
  • Neuroprotection

Dosing

5mg per day SubQ.

Reconstitution

10mg + 1mL BAC = 10,000mcg/mL. 30mg + 3mL BAC = 10,000mcg/mL.

Cycle Length

Daily use is studied. Often cycled with MOTS-c.

Timing

Morning SubQ injection.

Stacks Well With

MOTS-c, NAD+, Epitalon

Cautions

Do not combine with HGH gels (interference). Well-tolerated otherwise.

FOXO4-DRI

FOXO4-p53 interfering peptide

Mechanism

Senolytic peptide that disrupts the FOXO4-p53 interaction that keeps senescent (zombie) cells alive. Forces apoptosis (programmed cell death) in senescent cells while leaving healthy cells unharmed.

Benefits

  • Selective elimination of senescent cells
  • Physical rejuvenation in animal models
  • Improved fitness and fur density in aged mice
  • Reduced SASP (senescence-associated secretory phenotype)

Dosing

5mg EOD or 3x/week. Short cycles: 10 days on, 2 weeks off.

Reconstitution

10mg + 1mL BAC = 10,000mcg/mL.

Cycle Length

10-day cycles, 4–6 times per year.

Timing

Any time SubQ.

Stacks Well With

Epitalon, NAD+, MOTS-c (comprehensive longevity stack)

Cautions

Senolytic activity is powerful — some researchers experience temporary fatigue/malaise as senescent cells clear. Start with lower dose. Not for use with active cancer treatment.
